Medical Release: I ___________________________________ certify that my child/ I (if over 18) am in good health for participation in the 2026 Penfield Classic Field Hockey Tournament and my child/I will not hold coaches, officials, directors, or organizers responsible for any injury or illness that my child/I may incur, and that my child/I will wear the appropriate safety gear including shin guards and a mouth guard in ALL games and turf shoes or sneakers (no cleats) when playing on the turf field. I further certify that if my child/I am a goalie that my child/I will be wearing appropriate goalie equipment for ALL games.

CODE OF CONDUCT: Conduct should be above reproach, on and off the field, among and between players (opponents and teammates), officials, parents and fans. Unacceptable conduct or behavior from players or fans will not be tolerated and may result in disqualification from the tournament.
